The
Microsoft Word format allows by the youth worker to edit the forms if you
want to personalize them for your youth groups. Usually, it is a nice
touch to add your youth group logo and other information on the forms.
However, it is not essential.

Adobe Acrobat format uses the free program you can download
here.
This type preserves formatting and has also been set up so that the youth
can fill out the form on their computer by typing in the blanks.
